Responses are in green ink. 1st heading, the Name of the person (Muhammad Mahmud Ali Abu'l-Zaid); 2nd heading asks for religion, place and date of birth, which elicits the response, al'Islam, QP, 1933. 4th heading asks for names of children [Shahira (l O), Mariam (8), and Nafuz] (5). 6th heading concerns a trip. At bottom, three Israel revenue stamps, each 10 mils value.
